Assistive Technology Referral

NTFE has a set of Google Forms to get our Assistive Technology Evaluation and Recommendation Report Started. Please complete the forms that are pertinent to your student/child after contracting with NTFE to complete an Assistive Technology Referral. Additional Assistive Technology Inventory: Recreation and Leisure

If your AT Evaluation will include recommendations related to independent recreation, leisure and play, please complete this form. 

Additional Assistive Technology Inventory: Vision

Please complete this form if your student/child has a visual impairment. 

Additional Assistive Technology Inventory: Hearing

If your student/child has a hearing impairment please complete the hearing inventory form. 

Additional Assistive Technology Inventory: Computer Access

Fill this form out in addition to AT Referral and any other inventory areas if computer access and use will be included in AT Evaluation.

If your AT Evaluation will include recommendations related to executive functioning needs of your student/child.
